Empower360 has two plan options to meet your needs and budget - Essentials and Prime. These plans are designed to provide direct primary care in Western and Central Pennsylvania with local doctors.
ESSENTIALS PLAN
Our ESSENTIALS plan is a budget friendly option that provides our Member Exclusive and Standard Services. It's a great option for people who are in good general health but still want the assurance of receiving great medical care when they need it, and want to take a proactive approach to getting and staying healthy. You'll get a full range of testing and preventive care, as well as access to our full RX Benefit.
ESSENTIALS PLAN PRICING:
- Individual: $79/month
- Individual +1: $158/month
- Family (capped at 3 people): $237/month
PRIME PLAN
Our PRIME is our full featured option that delivers our Member Exclusive and Standard Services as well as adding our Advanced Services. It's the best choice for people who looking for a plan that offers access to specialties like cardiology and physical therapy or who may need ongoing care for chronic conditions or special medical equipment.
PRIME PLAN PRICING:
- Individual: $159/month
- Individual + 1: $318/month
- Family: $477/month
